This agreement regulates the Patient's access to services through the AZHealth Platform, the rights and obligations of the parties, payment, privacy and liability terms.
This agreement becomes legally binding when accepted electronically by clicking the "I have read and accept" button, in accordance with the Law of the Republic of Azerbaijan on "Electronic Signature and Electronic Document".

Platform owner: AZERBAIJAN HEALTH TOURISM MMC. Patient: A natural person who accesses, registers on and/or uses the services of the Platform. For persons under 18, the agreement is accepted by a legal representative.
AZHealth does not provide medical services. AZHealth is solely a technological intermediary/coordinator platform. Medical diagnosis, treatment plans and their outcomes are entirely the responsibility of the clinic/specialist providing the service.
Platform: www.healthtourism.az and related service modules. Clinic: An institution operating under the license of the Ministry of Health of the Republic of Azerbaijan. Personal data: Any information relating to an identified or identifiable natural person.
The Patient must provide accurate information. The Platform may require identity-verifying information. The Patient is responsible for the confidentiality of their account access credentials.
Information provided on the Platform is of a general informational nature and does not replace medical advice. In emergency situations, the Patient should contact the nearest Emergency Medical Service (103).
The Patient can create appointments through the Platform. Camera/microphone permission is required for online consultations. If the participant does not start within 10 minutes, the Patient may send a notification.
The Patient has the right to: be fully informed, refuse treatment, obtain copies of medical documents, file complaints, and access their personal data.
The Patient is obligated to: keep registration information accurate, fully disclose medical history, join appointments on time, and fulfill payment obligations.
Medical information is confidential. Audio/video recording of consultations without consent is prohibited. Correspondence is retained solely for the purpose of service execution.
AZHealth collects personal data directly from the Patient and during Platform usage. Medical documents are transferred to the clinic only with the Patient's consent. The Patient has the right to access and request correction of their data.
Registration on the Platform is free. Payments are processed through a licensed bank. Cancellation 24+ hours before appointment: full refund. Less than 24 hours: no refund.
Use of the Platform for illegal activities, forged documents, technical damage, offensive content, and fake reviews is prohibited. Violations may result in account suspension.
Medical liability belongs to the clinic providing the service. AZHealth's technical failure liability cannot exceed the amount paid by the Patient in the last 3 months.
Complaints are sent to info@healthtourism.az. AZHealth sends an acknowledgment within 5 business days and a response within 15 business days.
Notifications are sent via the registered email/phone. Agreement changes are notified 30 days in advance.
The agreement takes effect from the moment of electronic confirmation and is indefinite. The Patient may terminate the agreement at any time by deleting their account.
This agreement is governed by the legislation of the Republic of Azerbaijan. Disputes are resolved through 15 days of negotiation, then through the courts.
If any clause is deemed invalid, only that clause becomes void. The latest version is always available on the Platform.
The following choices are independent, each is accepted separately:
A — Main agreement (mandatory)

B — Commercial/marketing notifications (optional)
I agree to receive notifications about AZHealth campaigns and service updates. I can withdraw this consent at any time.
C — Analytics (optional)
I agree to the use of my usage data for anonymous analytics to improve the platform experience.
By clicking the "I have read and accept" button, I accept this agreement electronically. This electronic confirmation has the same legal force as a handwritten signature.
